There were various forms to fill out to receive a DP identity card. In this document, the IRO Welfare Officer noted the information that was later transferred to the identity card: name, sex, birth date, features such as hair and eye color, and nationality. Since this was an identity card for a newborn, the weight and height were not entered.
DPs had to submit passport photos and other documents as well as the pre-printed applications. The collecting officers (who were IRO employees) gathered these together and noted this on the application for an IRO identity card. The issuing officer who handed out the new DP identity card wrote the card number on the application. Jakob Zwikielberg received his identity card the very same day in Steyr.
